I, __________________________ (First Name, Last name), acknowledge and agree to the following:
 

1. Assumption of Risk: I understand that participating in sports activities at Keystone Sports Facilities involves inherent risks including, but not limited to, physical injury, mental trauma, or property damage. I voluntarily assume full responsibility for any such risks.

2. Release: In consideration of being allowed to use the facilities and participate in activities at Keystone Sports Facilities, I hereby release, waive, discharge, and covenant not to sue Keystone Sports, its owners, officers, agents, employees, or any associated parties (collectively referred to as "Releasees") from any and all liability for any loss or damage, and any claim or demands on account of injury to the person or property or resulting in death, whether caused by the negligence of the Releasees or otherwise, while I am in, on, or about the premises or while using the facilities or equipment provided by Keystone Sports.

3. Indemnification: I agree to indemnify and hold harmless the Releasees from any loss, liability, damage, or cost they may incur due to my participation in any activity at the facility.

4. Medical Treatment: I hereby consent to receive medical treatment which may be deemed advisable in the event of injury, accident, and/or illness during my activity at Keystone Sports Facilities.

5. Photography Release: I grant permission to Keystone Sports to use my photograph, video, or likeness for promotional purposes without payment or approval rights.

6. Understanding: I have read this release of liability and assumption of risk agreement, fully understand its terms, understand that I have given up substantial rights by signing it, and sign it freely and voluntarily without any inducement.
